篇名 富含醯麩胺酸之全靜脈營養對維持正常老鼠小腸黏膜細胞的影響

中文摘要        本研究主要探討在全靜脈營養(total parenteral nutrition,TPN)輸入不同熱量,造成不同程度之正氮平衡時,腸黏膜細胞萎縮的情形是否會有所改善,以及醯麩胺酸(glutamine,GLN)之添加是否會對腸道及腸黏膜細胞有所影響。實驗將老鼠做TPN手術後分成四組,一組對照組,三組實驗組,對照組餵老鼠飼料及僅打生理食鹽水,三組實驗組不由口進食,完全由全靜脈營養方式供給營養素及熱量,每100公克體重分別輸入25卡、30卡及35卡。各組實驗組再分為添加GLN與不添加GLN組,GLN添加組是以GLN取代胺基酸總量之40%,所有實驗組除了以葡萄糖調整成不同的熱量之外,其餘營養素含量及含氮量均完全相同,整個實驗為期六天。結果顯示在本實驗條件下,正常老鼠腸道及腸黏膜細胞重量並不因GLN之添加而有所差異,同時腸黏膜細胞之萎縮亦不因氮平衡之程度而有所不同。
英文摘要        The study was designed to investigate the effect of glutamine supplementation with different levels of caloric intake on the preservation of proximal jejunum mucosa in rats receiving total parenteral nutrition (TPN). Rats were divided into four groups; a control group was fed a chow diet and infused with saline only. Three experimental groups received TPN solutions at energy levels of 25 kcal, 30 kcal, 35 kcal / 100g body weight, respectively. The experimental groups were mintained with TPN for a period of 6 days. Each experimental group was divided into two subgroups, one of which was supplemented with glutamine, replacing 40% of the total amino acid nitrogen. All of the basal TPN solutions were insonitrogenous and identical in nutrient composition, except for the difference in energy level, which was adjusted with glucose. The results demonstrated that nitrogen retention increased in accordance with the increase of glucose supply. Howerer, no difference was observed in intestinal mucosa weight among the groups with different levels of caloric infusion. There were no differences in proximal jejunum segment and mucosa weight between subgroups with and without glutamine supplementation. It was concluded that higher nitrogen retention had no preventive effect on intestinal mucosa atrophy, and glutamine enrichment of a TPN solution did not have any effect on mucosa maintenance in normal rats.
